Step 3: You will have an intake interview with a clinician to set your treatment plan (90 min session)
We do not accept insurance. We want to dictate your plan of care and evaluate what is best for you.
You may however be eligible for reimbursement if you submit our invoice to your insurance provider. Here’s how:
Step 1: Call your insurance provider and ask what your ‘out of network benefits’ are
Step 2: You may provide the following codes:
- First Session (Intake): 90791
- Individual Therapy: 90834
- DBT Skills Group : 90849
- We can provide a specific diagnosis code after we complete an evaluation and begin services. If your insurance requires a diagnosis code upfront, you can use the general code F41.1
Step 3: You may provide our clinic codes:

We ask for payment information upfront and charge after each session
We accept cash, check or credit card (Visa, MasterCard or American Express)
Individual Therapy: No fee for cancellation in advance of 24 hours
Group Classes: Fee for cancellation (but we provide access to taped recording of the class)
Admin: Open 9 – 5 pm
Clinicians: Offer appointments all day, including evenings. Closed on weekends
We are providing full services via video and phone
We are located on the first floor (right off the lobby) of the 5000 Building in the Carillon Point Complex.
As you enter the Carillon Point complex, you will come to a four-way stop. Turn left and proceed to the parking garage at the end of the road on the left. You will receive one and a half hour free parking validation if you stop at Starbucks or use the restaurants at Carillon Point.
